Nothing on this site is clinical, legal, billing, coding or financial advice, and nothing on it creates a clinical relationship with any patient or a contractual relationship with any organization. Services are provided only under a written agreement.
Information on this site does not guarantee reimbursement, coverage, claim approval, patient eligibility, clinical outcomes or financial results. Descriptions of care-management programs are general; coverage and requirements are set by payers and applicable rules, change over time, and are the responsibility of the billing organization.
Clinical direction
Services described here operate under the clinical direction of the contracting organization. That organization retains clinical decision-making, practitioner oversight, and all billing and claims responsibilities.
No patient information
Do not submit protected health information through this website. See our Privacy Policy.
