Descriptions of care-management programs, billing codes and regulatory requirements are general summaries. Program rules are set by the Centers for Medicare & Medicaid Services, by other payers and by applicable federal and state law; they change, and they are applied to the specific facts of a practice. Confirm current requirements with your own counsel, compliance officer or billing advisor before acting on anything you read here.

2. No professional relationship until a written agreement is signed

Reading this website, submitting the contact form, exchanging email, or speaking with us does not create a professional, contractual or clinical relationship of any kind.

No services are performed and no obligation arises on either side until a written services agreement has been executed by both parties. Where the work involves protected health information, a business associate agreement is executed before any protected health information is exchanged. Until both documents are signed, any discussion of scope, timing or fees is exploratory.

Anuvra Clinical Solutions provides no clinical services to patients directly and holds no patient relationship. Services are provided to contracting healthcare organizations, under the clinical direction of those organizations.

3. Scope is limited to the executed agreement

What Anuvra does for a client organization is defined entirely by that organization's executed agreement: the patient populations in scope, the workflows and protocols approved by the practice, the escalation pathways, the documentation standards, and the responsibilities assigned to each party.

Nothing described on this website expands that scope. A capability described here is not a commitment to perform it for any particular organization, and services not named in an executed agreement are not part of an engagement.

The contracting organization retains clinical decision-making and practitioner oversight, and retains all coding, claim submission and payer-facing responsibilities. Assigned Anuvra staff work within protocols the practice has approved, and anything requiring clinical judgment beyond those protocols is escalated to the practice for a decision.

4. No guarantee of results

Anuvra Clinical Solutions makes no guarantee, representation or warranty regarding reimbursement, coverage, claim approval, patient eligibility, audit outcome, clinical outcomes, patient engagement rates, financial results or revenue of any kind.

Coverage, coding, claim submission and every payer decision are the responsibility of the billing organization. Where Anuvra's documentation supports a program the practice bills, that is a consequence of the work being performed and recorded properly. It is not a promise that any claim will be paid.

Any illustration, example workflow, figure or scenario on this website is illustrative only. It does not describe a specific client, a specific patient or a specific financial result, and it should not be read as a projection for your organization.

5. Limitation of liability

This website is provided on an "as is" and "as available" basis. To the fullest extent permitted by Ohio law, Anuvra Clinical Solutions disclaims all warranties relating to this website, express or implied, including warranties of merchantability, fitness for a particular purpose, accuracy and non-infringement.

To the fullest extent permitted by law, Anuvra Clinical Solutions and its officers, employees and contractors are not liable for any direct, indirect, incidental, consequential, special or exemplary damages arising from use of this website or reliance on its content, including lost profits, lost revenue, denied claims or business interruption, even if advised of the possibility of such damages.

This section limits liability arising from this website only. Liability arising from services performed for a client organization is governed by that organization's executed agreement, and nothing here limits or modifies it. Nothing in this section excludes liability that cannot be excluded under applicable law.

6. Third-party links and services

This website links to third-party services, including our scheduling tool and the provider that delivers messages submitted through the contact form. Those services operate under their own terms and privacy practices.

Anuvra Clinical Solutions does not control third-party content and does not endorse, warrant or accept responsibility for it. A link is not a recommendation. Where a third party handles information on our behalf in the course of providing services to a client organization, that relationship is governed by the applicable written agreement rather than by this website.

7. HIPAA and protected health information

Do not submit protected health information through this website. The contact form, our published email address and our published phone number are general business channels and are not secure channels for patient information. Do not send patient names, dates of birth, medical record numbers, Medicare or member identifiers, diagnoses, medication details or any other identifiable patient information through them.

Where an engagement requires access to protected health information, Anuvra acts as a business associate of the contracting organization. A business associate agreement is executed before any protected health information is exchanged, and the safeguards, permitted uses, breach-notification obligations and return-or-destruction requirements of that agreement govern the handling of that information.

The contracting organization remains the covered entity. It retains responsibility for its own compliance obligations, for the lawfulness of the disclosures it makes to Anuvra, and for its own notices of privacy practices. See our Privacy Policy for what this website itself collects.

8. Right to modify

Anuvra Clinical Solutions may change, correct, suspend or remove any part of this website, including this disclaimer and the descriptions of services on it, at any time and without notice. The effective date at the top of this page indicates when it was last revised, and continued use of the website after a revision indicates acceptance of it.

A change to this website does not amend an executed agreement. Amendments to an engagement are made in writing between the parties to that agreement.
